• Application for Admission (PDF file) - deadline January 15, 2011 (or turned in at Open House.
• Preregistration for Entrance Examination (PDF) - deadline January 25, 2011. (This form also contains School Records Request Form.)
• School Records Request Form (PDF file)
• Recommendation Forms (PDF files) - deadline February 1, 2011: English Teacher, Math Teacher, Principal/Counselor (These recommendation forms are the common recommendation forms for all Catholic High Schools. You need to sign only one form and then copy it and submit it to each of the Catholic schools to which the student is applying.)
• Financial Aid Application Deadline February 1, 2011.
