School & Athletics Physicals
Student athletes are required to have a new physical prior to every academic year, dated after April 15, and have their Magnus Health account completed by the end of June, in order to be eligible for participation.
Student athletes are able to upload their completed physicals once their Magnushealth.com accounts are active in early June. Please do not turn your physical form into the school or athletic offices.
Student athletes will upload their completed physical forms, download the required forms, and complete a brief online questionnaire.
Student athletes who elect to see a personal physician for their physical exam, click here for the STH Physical Form and then upload the completed documents to your Magnus Health account.
